The RECOVER-NEURO Cognitive Dysfunction (BrainHQ, PASC-CoRE, & tDCS) clinical trial tested 3 non-drug treatments to see if they improved thinking, focus, and memory for people with Long COVID. The 3 treatments included:

  1. An interactive online brain training program called BrainHQ.
  2. A virtually delivered, small group cognitive rehabilitation program called PASC-Cognitive Recovery or PASC-CoRE.
  3. A cap that provided a non-invasive form of brain stimulation via electrical current called transcranial direct current stimulation or tDCS.

A unique aspect of RECOVER-NEURO was its decentralized design, enabling 328 adults across 22 U.S. sites to participate in this research despite living with Long COVID symptoms. The participants completed most study activities remotely during a 10-week study period. Some participants were assigned to a treatment group and received one of the three active treatments (BrainHQ, PASC-CoRE plus BrainHQ, or tDCS-active plus BrainHQ). Other participants were assigned to a comparison group and received either the BrainHQ active comparator or tDCS-comparator plus BrainHQ. Researchers found that all of the treatment groups and comparison groups reported similar results in their improvement in thinking, focus, and memory. All participants reported some improvement over time and many said they felt better overall after they completed their assigned treatment, even though no one treatment outperformed other groups, including the comparison groups.


The researchers and study team running the RECOVER-NEURO clinical trial published a paper detailing the study’s design. RECOVER-NEURO is looking at 3 possible treatments for cognitive dysfunction, or brain fog, symptoms related to Long COVID. The possible treatments, also called study interventions, are BrainHQ (an interactive online brain training program), PASC-Cognitive Recovery (an online goal management training program), and transcranial direct current stimulation (a safe, noninvasive form of brain stimulation). The research team is using a variety of assessments, including brain function tests and participant surveys, to determine if these study interventions reduce brain fog symptoms and help people function better.

People with brain fog may have trouble thinking clearly, remembering things, or focusing on tasks, which can severely affect their daily functioning and quality of life. Results from this study will help the Long COVID community learn if BrainHQ, PASC-Cognitive Recovery, and transcranial direct current stimulation can be used to treat cognitive dysfunction symptoms related to Long COVID.

RECOVER researchers reviewed ongoing, planned, and potential future treatment options for Long COVID, based on our current understanding of the biological causes of Long COVID symptoms and conditions. The purpose of this study is to help guide future research by summarizing the current state of the science as a call for urgent and efficient research to help the millions who continue to suffer from Long COVID around the world.

Researchers identified a number of studies that attempted to treat the various symptoms and conditions of Long COVID that may be useful to consider or incorporate when designing a larger research project aimed at treating the underlying biological cause of those symptoms and conditions.
